The mission of MedLink Georgia is to provide quality, comprehensive, individualized diabetes self-management education to the patients we serve. we believe that education is the key to empowering the person with diabetes to better manage his or her disease and avoid the complications of diabetes and achieve an optimum health status. MedLink Georgia places a special emphasis on offering access to diabetes education to the underserved and high-risk populations in our community who are impacted by diabetes and aligns with our vision of “Healthy Individuals, Healthy Families, And Healthy Communities”. Our nationally accredited program provides education, training, and support to patients diagnosed with type 1 and type 2 diabetes, Maturity Onset Diabetes in Youth (MODY), Latent Autoimmune Diabetes in Adults (LADA), Gestational Diabetes, Impaired Glucose (Pre-Diabetes) and Metabolic Syndrome. Those referred to our program will partner with the registered dietitian and/or the diabetes care and education specialist to develop an individualized plan that encourages and supports the patient in adopting healthy lifestyle choices. Our educators will work with the patient, the patient’s family, and the patient’s physician to prevent long-term complications, improve diabetes management and blood glucose levels, and improve the quality of life. To participate in this amazing program, a physician’s referral is needed.

What is Diabetes Education?

Diabetes education services provide information and skills for people to manage their diabetes and related conditions. This program is tailored to your individual needs, goals, family history, and life experiences and is guided by evidence-based standards.

Additional Services We Offer

Nutrition Counseling

Our registered dietitian partners with our diabetes educators to provide assistance with meal planning, shopping on a budget, managing caloric intake and so much more!

RetinaVue® Scans

Our new scanners make it even easier for diabetic patients to access recommended annual diabetic eye exams. Offering these in our Family Health Centers provides a more convenient way for our patients to stay on top of their eye health.

Mental Health

Our Licensed Clinical Social Workers provide behavioral health therapy for individuals, children and adults. Our LCSWs work as part of an integrated care team along with MedLink medical providers and other specialty providers to offer quality patient-centered care.
