Tamara Herman, MS, RD, LD, CDCES

Service: Nutrition

Tamara S. Herman is a Registered and Licensed Dietitian, as well as Certified Diabetes Care and Education Specialist. She received her Undergraduate Degree in Dietetics from the University of Georgia and went on to complete her Dietetic Internship and her Master of Science in Clinical Nutrition from New York Institute of Technology. Tamara has been working in the field of Nutrition for 25 years, and in Diabetes Education for 20 years as a Certified Diabetes Care and Education Specialist. She has been employed at MedLink since 2022 and sees patients of all ages. She is able to assist with Diabetes, Weight Loss, Disorders of the GI tract, including enteral and parenteral nutrition requirements, Failure to Thrive in adults and pediatrics, Diagnosing Malnutrition, Nutrition for Renal and Hepatic disorders, cardio vascular disease, peripheral vascular disease, Stroke, Nutrition during pregnancy, food allergies, including Celiac Disease, and tips on healthy cooking. She is also able to teach patients about, but not limited to, diet, exercise, glucose monitoring, including Continuous Glucose Monitoring therapy, pathophysiology of Diabetes, and insulin start, via injection or via insulin pump, and education on other oral medications and non-insulin injectables.

Jordan Hoffman, LCSW

Service: Behavioral Health

Jordan is a Licensed Clinical Social Worker and is the Director of Behavioral Health for MedLink Georgia. She attended school at the University of Georgia, where she received her MSW. She has been employed with MedLink since 2017. She sees patients ages 4+ for a variety of counseling and behavioral health therapies including Dialectical Behavior Therapy (DBT), Acceptance & Commitment Therapy (ACT), and Eye Movement Desensitization and Reprocessing (EMDR).

Heather Jones, FNP-C

Service: Primary Care

Heather is a certified Family Nurse Practitioner at our White office. She earned a Bachelor of Science in nursing at Clayton College and State University and completed her Master of Science in Nursing degree from the University of Alabama at Birmingham. She completed her residency with the NoGAPP Family Nurse Practitioner Residency Program. She began working for MedLink in 2022 and sees patients of all ages for their primary healthcare needs.

Melissa is a certified Family Nurse Practitioner at our Suches and White offices. She attended the University of North Georgia and began working for MedLink in 2020. She sees patients ages 5 and up for their primary healthcare needs. She also provides preventive care, disease management, Hepatitis C treatment, wellness exams/physicals, and sports physicals.

Tonya is a certified Family Nurse Practitioner at our White office. She attended the University of North Georgia began working for MedLink in 2019. She sees patients 8 years and older for their primary healthcare needs. She also sees patients for basic women’s health services including contraceptive management, IUD and Nexplanon removals, HIV PEP/PrEP, Department of Transportation Physicals, Sport physicals as well as counseling patients for weight loss management.
