MedLink Georgia is a non-profit, Federally Qualified Health Center offering discounts to eligible patients for primary health care services as well as behavioral health, dental, podiatry, and pharmacy. MedLink Georgia is not a free clinic, however, as a Federally Qualified Health Center, we are able to offer uninsured and underinsured patients a sliding fee scale. No one is denied services based on inability to pay.

What is the Sliding Fee Scale Discount Program?

As a Federally Qualified Health Center, MedLink Georgia offers eligible patients discounts on their services. The fee discount opportunity is based on Federal Poverty Guidelines and is calculated based on total household size and income.

Am I Eligible & How Do I Apply?

If you are a patient without health insurance or if you are underinsured, you may be eligible. Ask a MedLink receptionist for the sliding fee scale application when you check in for your appointment. If approved, your discounted rate is locked in for one year. Applications must be completed annually.

In addition to filling out the application form, patients will need to provide proof of income.

What can be used as proof of income?

Any of the following documents are acceptable income verification documents:

  • Paystubs (from most recent 4 weeks)
  • Federal/State income tax forms
  • Food stamp eligibility notice
  • Letter from DFACS/SSI
  • Department of Labor quarterly report
  • Workers’ compensation award notification
  • Proof of income for all family members is required.

We realize patients occasionally have unique circumstances regarding income & housing. The Office Manager can provide you with further information & details to best help you complete our application process.

Frequently Asked Questions

For the purposes of MedLink’s Sliding Fee, household size includes:

  1. Patient
  2. Legally married spouse
  3. Domestic partner
  4. Natural, legally adopted and stepchildren, under the age of 19 (depending on residence)
  5. Children (as above) ages 19-26 if registered as full-time student
  6. Elderly parents/grandparents who are dependent on patient or spouse for support & are claimed as dependent on tax returns
Possibly! The Sliding Fee Scale Discount Program is calculated on total household size & income.

The minimum fee for a medical office visit is $40. Other services, such as dental, have a higher minimum fee. Your front desk receptionist will be able to give you pricing details before your service.
